Final Forms

Good afternoon,
Due to a late update in the "Forms" section of Final Forms there are 2 additional signatures that will be need to be signed. Many of our students who participate in a fall sport/group most likely had their forms completed prior to the addition of these 2.  I apologize for the inconvenience but I am asking that you log on over the weekend to sign off on these 2 (and others if necessary) before your student begins school next week.  Please use the picture below for your reference.
